Apply online right from your browser. Treatment with a special shampoo and removal of ALL nits is required before the child returns to school. Parents are requested to send the box top from the shampoo to the school clinic when the child returns. Students with Diabetes - Reminder...... Parents of students with diabetes,should get an updated plan for school from the healthcare provider when they visit the doctor. This will be needed for next school year .
